At this time, the Health Center of Helping Hands does not have the Covid19 Booster vaccine. Please contact your physician or go to Vaccine.org for the locations that currently have the vaccine.
Vaccination Card Replacement:
If you received your Covid19 vaccine from the Health Center, please go to the clinic and they will issue a new card. 401 W Rusk Suite 100 Rockwall, TX 75087. (don’t give a phone #)
If you did not receive your vaccine from the Health Center of Helping Hands, please follow the directions below for a replacement card.
Request an Immunization Record
To request an immunization record for yourself or your child, fill out the Authorization to Release ImmTrac2 History form. Those who are 18 years old or older must sign an ImmTrac2 Adult Consent Form (EF11-13366). To maintain immunization records in the registry, they must submit the form by their 26th birthday.

- Complete the form.
